Best Practice Support Service: Academic Detailing Feasibility Study

Building on relationships of trust to deliver unbiased, evidence-based information to clinicians.

CEP designed, implemented and evaluated the first large-scale feasibility study for academic detailing in Ontario. Based in Sudbury and the Greater Toronto Area, this project focused on type 2 diabetes management. This program trained skilled health care providers to deliver independent, evidence-based information to providers and included a detailed evidence document, clinical tools and patient handouts.

Description

From September 2007 to 2008, CEP developed and implemented an academic detailing pilot program focused on two geographic areas: Sudbury and the Greater Toronto Area. The clinical focus of the Best Practice Support program was on difficult aspects of type 2 diabetes including insulin starts, role of glitazones and the prevention of complications.

Expert academic detailing service facilitators deliver independent advice and evidence-based information to health care providers in their own office setting during brief one-to-one encounters. Specific clinical topics of interest to practitioners are discussed and ongoing support is provided targeting positive patient-care outcomes and good value in healthcare delivery.

Elements of this program included:

  • Partnerships with Med-I-Well Services, the Ontario College of Family Physicians, Ontario Pharmacists’ Association, Nurse Practitioners' Association of Ontario, Hamilton Family Health Team, Centre for Evaluation of Medicines Drug & Therapeutics Information Services (DATIS, Adelaide, Australia), Independent Drug Information Service (iDiS, Pennsylvania)
  • Recruitment of academic detailers who are affiliated with Med-I-Well Services, Summerville Family Health Team and Barrie Family Health Team
  • Content development pertaining to clinical area (e.g. evidence summaries, identification of key messages and enablers)
  • Development of performance aids to promote best practice (e.g. algorithms, patient management flow sheets)
  • Development of patient education tools to support best practice
  • Design of an evaluation framework
  • Development and hosting of a training program (e.g. workshops, teleconferences, materials, etc.) for academic detailers
  • Development of a comprehensive promotion and recruitment plan
  • Collaboration and linkages with other diabetes educational programs
  • Roll out to clinicians in Sudbury and the GTA (180 visits)
